What the record should do
- Show all the provision the school makes that is additional to and different from what is offered through the curriculum.SEND Code of Practice 6.76
- Record the provision made for pupils with SEN accurately, and keep it up to date.SEND Code of Practice 6.72
- Record the details of additional or different provision made under SEN support, so you can evidence the support provided over the pupil's time in school, and its impact.SEND Code of Practice 6.73
- Be ready to share it. A local authority considering an EHC needs assessment will want to review this information.SEND Code of Practice 6.73
- Share it with parents in an accessible format — for example, the highlighted rows of the provision map for their child.SEND Code of Practice 6.75
The provision map
One row per provision. Add the cost columns if your school costs its provision; the Code does not require them.
The four broad areas of need
- Communication and interaction
- Cognition and learning
- Social, emotional and mental health difficulties
- Sensory and/or physical needs
| Provision | Area of need | Pupils | 1:1, small group or in class | How often and how long | Who delivers it | Start date | Review date | Cost per week | Funding |
|---|---|---|---|---|---|---|---|---|---|
Keeping it current
Each term- Review each provision on its agreed date: is it having the expected impact on progress?SEND Code of Practice 6.53
- Use the broad areas of need to check how well equipped the school is to provide support across them.SEND Code of Practice 6.25
- Remember the areas give an overview of the range of needs to plan for — not a category to fit a pupil into.SEND Code of Practice 6.27
- Add an end date when a provision stops, rather than deleting the row. The map is the record of what was provided.Good practice — not a legal requirement
- Use the map to keep an overview of the programmes and interventions used with different groups of pupils.SEND Code of Practice 6.76
